Sometimes, a strong blow to the groin can cause twisting of the spermatic cord. At this time, the arterial blood flowing to the testicles can continue to supply blood to the testicles even though the pressure inside the twisted spermatic cord increases. On the contrary, the venous blood of the testicles cannot overcome this resistance and is retained in the testicles, causing the testicles to become congested and swollen. Dr. Shiber warns that if the congestion is not treated urgently within 4 to 8 hours, the testicles will "slowly decay and die." After a day or two, it will swell like a grapefruit; after 2 to 3 months, it will slowly shrink to the size of a pea - at this point, it is already a lifeless wreck. Dr. Siber urges that patients with severe testicular swelling, bleeding or bruising should immediately seek medical attention from a urologist or other physician to avoid lifelong regrets.

The Little World of Testicles

The testicles float freely at the end of the spermatic cord, like astronauts walking in the vast space. The so-called spermatic cord is made up of blood vessels, nerves and other tubes, and is responsible for connecting the testicles to the rest of the body.

The testicles are enclosed in a fluid-filled scrotum, rather than being in direct contact with the outside world and floating around at will.

If further subdivided, the testicles can be divided into 250 small units, each of which contains numerous seminal tracts that are twisted and tangled into a maze-like structure, and the seminal tracts are where sperm are produced. Once the sperm matures, it carries its magical genetic code into the seminal vesicle and epididymis. The epididymis is a small structure consisting of many thin tubes that spirals behind each testicle. Sperm reside here until orgasm, when they are ejaculated through the vas deferens. The vas deferens is the passage that connects the spermatic cord to the penis, and it is also the part that is cut when men undergo sterilization surgery.

Deep inside the testicles, Leydig cells are busy producing the male hormone, testosterone, which is both the fuse that ignites sexual desire and the main driving force behind the appearance of secondary sexual characteristics in men during puberty, such as beards, increased muscle strength, and a deepening voice. Although under normal circumstances, the two testicles of humans work together to synthesize sperm and testosterone and produce male secondary sexual characteristics, in fact, all these functions can be completed independently by one testicle - doesn't it sound interesting? Then why do we need to grow two testicles? Dr. Berg, professor of urology at the University of Washington and co-author of the book "Biocompetence: A Guide to Successful Sex," believes that "one of the testicles is a storage device."

Dr. Berg also explained: "In fact, the two major functional systems of the testicles, namely the testosterone production mechanism and the sperm production mechanism, are separate, and the latter is more fragile and more vulnerable to damage than the former." This is why some infertile men have healthy and perfect sex drives and do not have erectile dysfunction.

But this does not mean that the testosterone production mechanism can be harmed at will and does not need to be taken care of. For example, an epididymal infection, commonly known as epididymitis, is a common, extremely painful infection. The pathogenesis is usually retrograde infection of the urinary tract by bacteria into the epididymis. Because there are many tiny valves in the testicle that allow sperm to pass smoothly and go "up" into the epididymis; and these valves can also prevent the bacteria in the epididymis from roaming "down" into the testicle and destroying the testosterone production mechanism of the testicle. This is why we often see men infected with epididymitis whose epididymis is swollen and painful, but their testicles are intact and there is nothing wrong with them.
