Although penile fracture is not a common emergency, when it occurs, according to surveys, a general hospital receives an average of 6 to 9 cases of penile fracture per month. The patients are aged 20 to 40 years old, with an average of 32.5 years old, and the cure rate is 88.8%.

Why does the penis fracture?

The penis has two corpora cavernosa and one corpus spongiosum, which are surrounded by a tough white membrane. The corpora cavernosa are like a real sponge. When there is sexual stimulation, the blood flowing into the penis increases significantly, while the blood outflow decreases significantly, causing the corpora cavernosa to rapidly enlarge, the internal pressure to increase, and the penis to become hard and erect, just like a tire being inflated. After sexual excitement subsides, the veins in the penis open, blood outflow increases, the internal pressure drops, and the penis shrinks and softens, which is equivalent to deflating a tire. Of course, penile erection is far more complicated than the process of inflating a tire. It can only be completed when the penis structure is normal, the innervating nerves and blood vessels are normal, the male hormones are normal, and the mental state is healthy, and it is sexually stimulated. After the penis is erect, it becomes hard due to the high internal pressure, and is prone to rupture and breakage. Penile fracture is caused by this reason.

There are three main causes of penile fracture:

1. Wrong sexual method: During sexual intercourse, the erect penis is not aligned with the vaginal opening but violently hits the perineum or other parts, or using unsafe sexual postures and positions, causing the penis to bend forcefully for a moment when it is erect. This can lead to a fracture of the penis.

2. Penis trauma: When the penis is erect or semi-erect, penile fracture is caused by external gravity or blunt force.

3. Unreasonable masturbation behavior: Some people often hit or bend their erect penis with their hands to get stronger stimulation when masturbating. Others constantly insert their penis into certain objects in order to vent their sexual desire. In this case, penile fracture is likely to occur.

How to provide first aid when a penis is broken?

Since the main structures of the penis are arteries, veins and cavernous sinuses, massive bleeding will inevitably occur after the penis is cut off, which can lead to hemorrhagic shock or even death. Therefore, the first thing to do after the penis is cut off is to take emergency measures to stop the bleeding. If part of the penis is cut off, the base of the penis can be tied with a rope. If it is cut off from the base of the penis, pressure should be applied to stop the bleeding, using clean cloth such as towels and clothes. The second thing is the treatment of the severed penis. If it is properly preserved and treated in time, penis replantation can be performed. If the penis is not completely severed, it should not be cut off but kept in place because even if there is a little tissue connected, a small amount of blood supply is still preserved, which is also conducive to the survival of the severed penis. If the penis is completely severed, it should be cleaned, disinfected, and refrigerated immediately. It can be placed in a plastic bag, tied tightly, and placed in an ice bottle with ice cubes around it. Because storage at low temperatures can prolong the time it takes to deteriorate, if handled properly, successful replantation is still possible within 18 to 24 hours after injury.
