(I) Abnormal semen

1. Azoospermia or too few sperm

When the sperm density in semen is lower than 20 million/ml, the woman's chance of pregnancy is reduced. When it is lower than 20 million/ml, infertility will occur. This type of infertility can be divided into permanent and temporary. The former is seen in people with congenital testicular development disorders or severe testicular and seminal vesicle lesions; the latter is more common in people with excessive sexual intercourse that leads to a temporary failure of spermatogenesis, generally with a decrease in sperm rather than a total absence of sperm.

2. Poor sperm quality

Too many inactive or dead sperm in semen (more than 20%), or very poor sperm motility or more than 30% abnormal sperm can often cause infertility.

3. Abnormal physical and chemical properties of semen

Normal semen coagulates into a jelly-like state quickly after ejaculation, and then completely liquefies within the next 15 to 30 minutes. If semen does not coagulate after ejaculation or if liquefaction is incomplete, it often indicates that there is a lesion in the seminal vesicle or prostate. Bacterial and viral infections of the reproductive tract can also cause changes in semen composition and lead to infertility. Pathogenic bacteria exceeding 103/ml and non-pathogenic bacteria exceeding 104/ml in semen can cause infertility.

(II) Spermatogenesis disorders

1. Testicular disease

Such as testicular tumors, testicular tuberculosis, testicular syphilis, nonspecific testicular inflammation, testicular atrophy after trauma or spermatic cord torsion, testicular absence, etc., can all cause spermatogenesis dysfunction and infertility.

2. Chromosomal abnormalities

Sex chromosome abnormalities can cause poor differentiation of sex organs such as the testicles, resulting in true hermaphroditism and congenital testicular dysgenesis. Autosomal abnormalities can lead to metabolic disorders of gonads and spermatogenic cells.

3. Spermatogenesis dysfunction

Long-term consumption of cottonseed oil can affect spermatogenesis and sperm autoimmunity, and can also cause spermatogenesis dysfunction.

4. Local lesions

Diseases such as occult varicocele and giant hydrocele affect the local external environment of the testicles, or cause infertility due to temperature, pressure and other reasons.

(III) Sperm and egg combination disorder

1. Sperm duct obstruction

Such as congenital absence and atresia of the vas deferens, surgical ligation of the vas deferens, chronic inflammation of the vas deferens and surrounding tissues, etc.

2. Retrograde ejaculation

For example, if the bladder neck has undergone surgery or been injured, or scar contracture after surgery has caused urethral deformity, bilateral lumbar sympathetic denervation or abdominoperineal surgery for rectal cancer, pudendal nerve damage caused by diabetes, spermatic cord cyst hypertrophy, and severe urethral stenosis, certain drugs, such as adrenaline blockers, can cause changes in the function of the sympathetic nerves that control the bladder.

3. Abnormal external genitalia

Such as congenital penis deficiency, small penis, male pseudohermaphroditism, epispadias or hypospadias, acquired penile inflammation or injury, scrotal edema, giant testicular hydrocele, etc.

4. Male sexual dysfunction

Impotence, premature ejaculation, anejaculation, etc.

(IV) Systemic factors

1. Mental and environmental factors

The sudden change in living environment leads to long-term mental stress, high-altitude, high-temperature, ultra-intensive labor, and radiation work.

2. Nutritional factors

Severe malnutrition, vitamin A and vitamin E deficiency, deficiency of trace elements such as zinc and manganese, calcium and phosphorus metabolism disorders, chronic poisoning by toxic substances such as mercury, arsenic, lead, ethanol, nicotine, cottonseed oil, chemotherapy drug treatment, etc.

3. Endocrine diseases

Such as pituitary dwarfism, obesity, reproductive incompetence syndrome, adenohypophysis, congenital gonadal dysgenesis, congenital spermatogenesis syndrome, hyperprolactinemia, pituitary tumor or intracranial infection, birth trauma, etc.
